Community Health Services at 407 Independence Drive, Napoleon, Ohio.

The service itself was not bad. A quick physical, took 5 minutes of their time. The problem is with their information. When I first called, I was told it would be a $25 physical fee. Which would be great since that was slightly cheaper than every other place I had called. Paid my fee and went on my way. This was in August 2017. Now February 2018 I receive a letter stating I still owe for the physical. Which apparently is over $100. If I would have known that, I would have gotten a basic physical from anywhere else instead of dealing with this runaround. If you decide to go here, ask MANY questions so you know everything up front.
